Step to reproduce bug.  

 Login with I B-->Go with form "Addendum"-->Qus.no 10--> heading of the second column should be    "Percentage of holding"     instead of    Address. 

 Need to extend character length 200 "Nature of business field". 
                                 200 " Name of company"
