Step to reproduce the bug.
1. login with International Bank
2. Go with the form schedule 01
3. Qus.no 05 and 08
Point-->1.Provided document get disappear
Point-->2. In Qus.08 4th document get disappear
Point---> Applicant is not able to preview the document
