Step to reproduce bug. 
 1.Login with International bank 
 2.Go with the form "Schedule 1" 
 3.Qus.no    14 

 Actual Result: 1.    when the applicant adds a new Row applicant can fill 7 fields only the remaining field disappear.  
                 2. Table is not column wise.
