Step to reproduce bug. 

 1.Login with ERSP 
 2.Go with form "INFORMATION IN SUPPORT" 
 3.Fill the form and save 
 4.Qus.no 22 

 Actual Result: After save value enter on a charge field is not showing    it is showing Qus.13    Name field value on charge field. 


 other value.
