Step to reproduce bug. 
 1.Login with ERSP  
 2.Go with form "INFORMATION IN SUPPORT OF AN APPLICATION" 
 3.click action button. 
 4.Qus no. 10. 

 Actual Result:1. Result: form Question not match    document. 
               2.Right now Percentage field alphanumeric  
 
 Expected Result:1. Result: Question should be as per the client requirement same on form. 
                 2. Percentage field should be numeric.
