Step to reproduce bug. 
 1.Login with credit unions  
 2.Go with form "Previous Residence Details 
 3.Qus no. 32  

 Actual Result: Unselected all checkbox and save-->After save all checkbox show selected. 
 Expected Result: Information should update accordingly.
