Step to reproduce bug. 
 1.Login with NRSP client applicant 
 2.click on action button 
 3.Go with form "INFORMATION IN SUPPORT OF AN APPLICATION" 
 4.Fill the form and submit  
 5.Go with admin panel  
 6.License request-->Pre-license request. 
 7.question no. 13 table not updated as per the client requirement in admin panel. 
 8.question no. 22 provided value disappear in admin panel.
