Login with credit unions-->go with form "Application Details"--> Point 5-->only numeric value as per client comment in doc. Point no 9-->Implement as per client requirement.. Point no 10-->Implement as per client requirement.. Note: Modification as per client suggestion: Form Name: Application Details–> Point no-10→ 1. Remove checkbox from all 5 options . 2. Need to add upload option for document (Document upload should mandatory) Point 11-->Implement as per client requirement..
