When i have selected "Decline" in Insurance in form page of home shopping, in the pdf there is still "Accept" selected

Issue steps:
1. On the form page of home shopping select "Decline" in the Insurance
2. Navigate to home shopping account details
3. Download the customer form
4. Observe the selected option in Insurance
