Step to reproduce the bug:

After saving the provided values some input fields gets disappear and the 2nd dropdown value is also reset: see below.

1. Full Name of Licensee
2. Last Name
3. Address2
4. Details Input field
5. Shares and percentage*
6. 2nd dropdown value
