Login with I B-->Go with form "Addendum"-->Qus.no 7,15,21(a)(b) --> after saving value gets disappear

This issues has been verified.

1 issue are fixed.

2 issues Reopened--> 21 (a) (b) After saving selected value disappear.
--> 15 Yes No option after saving reset (unselected)

This issue has been verified.

2 issues Reopened--> 21 (a) (b) After saving Provided value disappear
after saving provided document disappear.
