Description

Step to reproduce the bug.

1. Login with International Banks
2. Go with the form "Addendum"
3. Qus.no 13,17,20,21,28

Point 1--> Need to extend character length Right now character length is maximum field 30.
point 2--> should be alphanumeric along with a special character (Name field should be alphanumeric)
