Description

Step to Reproduce bug.

1.Login with International Bank
2.Go with form schedule 01
3.Qus.no 04

Point-->1 date of the appointment field show 0000
Point-->2 Address, job title, nationality, Telephone, fax, email, field value disappear
point-->3 document value disappear
