Login with International bank-->Go with form "Addendum "-->Qus.no 28 -->when applicant add new Row applicant is able to fill 2 field only remining field disappear.

Description

Step to reproduce bug.

1.Login with International bank
2. Go with the form "Addendum "
3. Qus.no 28

Actual Result: when the applicant adds a new Row applicant can fill 2 fields only the remaining field disappears.
