Login with ERSP-->Go with form "INFORMATION IN SUPPORT OF AN APPLICATION"-->Click action button-->Fill the form and save -->After save provided document disappear on entire form.

Description

Step to reproduce bug.
1.Login with ERSP
2.Go with form "INFORMATION IN SUPPORT OF AN APPLICATION.
3.Click action button
4.Fill the form and save.

Actual Result: After save Provided document disappear on entire form.
Expected Result: provided document Should not be disappear on entire form.
