Step to reproduce bug.
1.Dev link
2.Login with ERSP client applicant
3.Go with form "schedule1"
4.click action button
5.Fill the form and save
6.Qus no.18

Actual Result: After save Sequence of rows from 1,2,3 change into row sequence 1,1,1.
Excepted Result: After save sequence of rows should be 1,2,3,4.
