Link test-->Login with credit unions-->Go with form "Member of applicant"-->Qus no.13-->for very first row--> click on the delete button---> clicking on delete button, model window for attachment gets open on the place of delete that particular row.

Note:
Delete functionality is working fine for second row, means issue is only for first row
