Step to reproduce bug.
1.Test link.
2.Login with client unions applicant.
3.Go with form "1.Documentation Checklist and 2.Documentation.
4.Fill the form and save.
Actual Result: after save form selected checklist automatic unselected and selected all option.
expected Result: selected checklist should not be change after save.
